To sign up for the nutrition program, register at Calvary Baptist Church, on January 23rd from 11:00am- 12:30pm, located at 1733 West Avenue C, Muleshoe, TX 79347.

Applicants Must Provide:
n ID or Driver License
n Proof of address/bill
n Must have Name and Physical address on it and must have been received within the last 30 days.
n Acceptable proof: Light, Gas, Water or Phone bill. (We CANNOT use your DL/ID as proof of address)
n Proof of Income. This can include Social Security Award Letter, SNAP benefits award letter, or SSI award Letter.
n For people 60 years or older.
n Monthly Delivery of a Food Box to Calvary Baptist Church, from the South Plains Food Bank.
n Income Guidelines by Monthly Income:
1 household member: $1,307 5 household members: $3,118
2 household members: $1,760 6 household members: $3,571
3 household members: $2,213 7 household members: $4,024
4 household members: $2,665 8 household members: $4,477
You must qualify to receive a food box. You must be present to register. You will not be allowed to register for anyone except yourself and people in your household. Everyone must reapply every 6 months. In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, religion, sex, gender identity (including gender expression), sexual orientation, disability, age, marital status, family/parental status, income derived from a public assistance program, political beliefs, or reprisal or retaliation for prior civil rights activity, in any program or activity conducted or funded by USDA (not all bases apply to all programs). Remedies and complaint filing deadlines vary by program or incident.
